Care Coordinator - 30 hours per week
We are looking for an organised and compassionate individual to take on a rewarding, varied role that combines essential administrative support with direct patient contact.
The Role: As a Care Coordinator, you will manage document workflows and prescriptions, support practice projects like Care Navigation, and assist with the Patient Participation Group. This role also offers the exciting potential to train in phlebotomy and assist with clinical duties.
About You:
- Strong administrative background with a keen interest in healthcare
- Excellent attention to detail and a sensitive approach to patient needs
- Adaptable team player ready to work within a multidisciplinary clinical team
Person Specification
Essential qualifications
- GCSE A - C (or equivalent) English
Desirable qualifications
- NVQ level 2 in health and social care
- Phlebotomy qualification
Essential experience
- Worked with Primary care, preferably general practice providing care navigation or social care providing care navigation
- Experience of working in a fast-paced environment, managing priorities.
- Experience of working with the public face to face and over the phone.
- Experience of working as part of a multi-disciplinary team.
Desirable experience
- Knowledge of SystmOne
- Working in a GP Practice for 2 years

Job Types: Part-time, Permanent
Pay: Up to £12.82 per hour
Expected hours: 30 per week (Monday to Friday)
