New Patient Registration
The practice welcomes new patients. Please give your NHS medical card to our receptionists or ask for an application form. You will also be given a form asking whether you wish to opt out of the Summary Care Record service.
When registering, patients are asked to complete a questionnaire to bring the GP up to date with their current state of health. For families with children we emphasize the particular importance of recording immunisations and vaccination history of children
The practice area serves the whole of Verwood and the surrounding areas of Gotham, Burrows Farm, Woodlands, Dewlands Common, Noon Hill, Ebblake, Potterne & Three Legged Cross, with the exception of Ringwood and Horton roads.
Medical treatment is available from the date of registration. Please contact reception for further information.
