Tracker Nurse Service

Sister Hilary Fuggle is our Tracker Nurse. The Tracker service has been set up to support our patients who are vulnerable patients over the age of 65. By vulnerable we mean anyone living alone, caring for somone, elderley and suffering from a debilitating illness or just not coping... anyone needing support and help which could cover medical, social and psychological.

The tracker service has managed to keep many patients out of hospital, thus freeing hospital admissions for more acute condistions. The Tracker nurse liaises with other agencies to provide care, lunches, aids for daily living, social services and physiotherapy and occupational health.

Anyone concerned about someone who belongs to the Practice can refer them to the Tracker Nurse Service by speaking to Marcia Russell, or any of the reception team. Some of the Tracker Nurse Service is carried out as advice over the phone, but home visits are undertaken if it's more appropriate.

Hilary Fuggle is also our Carers' Lead

Marcia Russell is administration and support for Hilary and can be contacted weekdays from 9.00am until 1pm and 2.15 until 3.30pm
